Canine Massage South Yorkshire acknowledges and respects the Veterinary Surgeons Act 1966 and Exemption Order 1962 by never working on a dog without gaining prior veterinary consent. This is to ensure that in the opinion of the vet, your dog is fit enough to have massage and that there are no contraindications to treatment.  A therapist who works on a dog without vet consent is doing so unlawfully and your dog could be at risk.

Please ensure that you have filled in the top half of the form prior to sending it to the vets and that the vets are aware of any changes in your dog's health.
